Being a new patient can be hard and overwhelming. We value your time and commitment to your health, and we want to ensure that you receive the best care possible. Please remember the following:

  • Cancellation Policy: Should you need to reschedule or cancel your appointment, kindly provide a minimum of 48 hours' notice. A no-show fee of $80 will apply if proper notice is not given.
  • Arrival Time: Please arrive promptly for your appointment. If you are running late, we kindly ask that you call our front office at 503-469-9818 to inform us. Please be aware that any delay may impact the duration of your appointment.
  • New Patients: Please bring a Photo ID and Insurance Card(s) with you at the time of your appointment. If you are using Insurance, please contact your insurance company with any questions regarding your coverage. If you decide to pay CASH, the New Patient appointment fee is $325 due at the time of service.
  • Our providers like to review this paperwork prior to your appointment. Please print out the paperwork at home, scan, and email back to Nutura Clinic prior your appointment at the following email address: info@nuturaclinic.com
  • If you have outside records you would like to have the doctor review, please send those as well to: info@nuturaclinic.com. If you are unable to print and complete the intake forms, please arrive 20 minutes early to allow sufficient time to complete.
